The first step in the admissions process is filling out an application and submitting it to Lake House Academy. This can be done on our online paret portal by clicking here, or a paper application can be filled out then returned by email, fascimile, or by mail; click here for the printable application. Once the application has been submitted you will be contacted by a member of our admissions team to assist in gathering all documents needed to begin processing an enrollment. These documents include your daughters academic transcripts, immunization records, along with any educational assessment(s) and/or testing, and any psychological evaluation(s) and/or testing.
Before we can enroll your child we need to have the appropriate documentation on file. This ensures the absolute best care for your child. Upon approval of your application you will need to complete the enrollment packet in its entirety and send back to us. Please keep in mind some of these documents will need to be notarized like the Lake House Academy Enrollment Agreement.
The enrollment packet will be sent once your daughter has been accepted to Lake House Academy. They can be sent by email, facsimile, or by mail. Please contact the admissions department at 877.267.5888 once you are ready to complete these.
